AGE UK are able to provide information and advice.  They are an independent charity working with and for older people.

SOCIAL CARE SUPPORT

If you or someone you know needs help with day-to-day living because of illness or disability it can be confusing to understand the options and where you can get support. Listed below are some resources you may find helpful if you or someone you care for needs social care or support to live independently.

Dementia Clinic at Bourn

The Alzheimers Society hold a bimonthly support clinic within the surgery where they are happy to patients and their carers to provide support around any form of dementia (not just Alzheimers).  They are a fantastic resource and can signpost you to a wide range of support and services.

If you would like to make an appointment, you can do so through our receptionists.

 

Carers Support

Please see dedicated page under Bourn Surgery Services.

If you are an informal carer for a patient (whether registered at Bourn or elsewhere) you are eligible for a referral to the Carers Trust for support as a carer.  You can be referred by any member of staff at the surgery, please ask your GP, nurse or receptionist.
