New Patient Registration

Bourn Surgery is currently accepting new NHS patient registrations for patients living within our practice boundary.

Please note we are a small surgery, at times there will be a limited choice of GPs working on any given day. Urgent appointments will be with the next available clinician rather than the GP of choice.

To register as a new patient at the practice. Please follow the registration process below.

Are you eligible to join the practice?

We are pleased to accept new patients from the following villages: Bourn, Caxton, Longstowe, Great & Little Gransden, Arrington, Kingston, Toft, Hardwick, Great & Little Eversden, Caldecote, and Comberton

If you are already a patient and are about to move to a new address please let the reception team know by completing our change of details form. We can only accept a patient from our named villages as above.

For identification purposes, you will need to upload proof of ID and proof of address at the end of the form. Proof of ID can be a passport or driving licence. Proof of address can be a utility bill (gas, electricity or water) or bank statement – dated within the last 3 months.

Temporary Services

If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.

To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.

Your Named GP

Each patient will have a named GP although patients are registered with the practice rather than an individual GP. You are welcome to see whichever clinician you prefer although we do encourage patients to book with the same GP where possible in order to provide continuity of care. Your medical card will be issued in the name of the surgery.

Change of Personal Details

Please notify the practice as soon as you can of any change of address or telephone number, so that your records can be kept up to date to ensure that reminders for blood tests, health checks, smears etc. go to your correct address. It is also important that we have your correct address and telephone number should the doctor need to make a home visit.

If you move out of the practice area the doctors can no longer be responsible for your health needs. We therefore advise you to register with a surgery in the area you live as soon as possible. Please ask at reception for further details.
