New Patient Registrations
Bourn surgery is currently accepting new NHS patient registrations for patients living within our practice boundary.
We are pleased to accept new patients from the following villages: Bourn, Caxton, Longstowe, Great & Little Gransden, Arrington, Kingston, Toft, Hardwick, Great & Little Eversden, Caldecote, and Comberton
You can register by bringing in your medical card or by completing a “form GMS1” available from reception
Each patient will have a named GP although patients are registered with the practice rather than an individual GP. You are welcome to see whichever clinician you prefer although we do encourage patients to book with the same GP where possible in order to provide continuity of care. Your medical card will be issued in the name of the surgery.
Please note we are a small surgery, at times there will be a limited choice of GPs working on any given day. Urgent appointments will be with the next available clinician rather than the GP of choice.

Due to the current lockdown we are happy to register patients remotely.
To complete your registration, please complete:
♦ GMS1 Form (see link below)
♦Bourn Surgery Health Questionnaire (see link below)
♦Provide one form of photo ID*
♦Provide proof of your address*

All registration documents must be emailed to the practice email: firstname.lastname@example.org
Please contact reception after 7 working days to check on the progress of your application; we should then be able to make a telephone appointment with a clinician if you have any urgent health needs or need repeat medications to be arranged.
