Online Access

There are 2 types of patient access:

Basic Access – book appointments online, order repeat medications and change your contact information. Also a basic summary of your records can be viewed on line this will include immunisations, allergies and a summary as well as medications.

Advanced access – All of the above PLUS viewing your medical record online this is called detailed coded record access and may be useful for certain patients who need to check results. The GP will need to check the records and agree that this would be useful to the patient. Please allow at least three weeks for this. The GP may decide that it is not in#

Please note – Online services are only available to patients aged 16 and over. Proxy access can be given to parents – but this will stop at 11yrs old. There is a form to be competed for this access to be granted. Please ask at reception.

Basic On line Access

Please complete the Basic Access Form for this service. You will need 2 forms of ID: one with a photo and one with your address (usually a utility bill, bank statement within the last 3 months). We require ID because you are able to view confidential information and this is to ensure you are the patient requesting access for safety.

You will be given a registration sheet by reception including a pin, the practice ID and your NHS number you will need this to register for online access.

Advanced Access

If you would like advanced access please ask at reception for a form and read the information leaflet

Once you have read the information leaflet, you will need to complete the advanced access form; this will then be looked at by GP and it can take 21 days for access to be seen.

If you are able to provide your mobile number and consent for SMS messaging we can text you when this has been done.

Safeguards have been negotiated in implementing expanded records access. There are circumstances where a GP may believe it is not in the best interests of the patient to share all information in the record, for example where it could cause harm to their physical or mental health. GP medical records software will be configured to offer all coded data by default, but GPs will be provided with the tools to withhold coded information where they judge it to be in the patient’s interests or where there is reference to a third party.
